Contrary to popular belief, Sailor Moon is actually considered a children's series in Japan. The merchandise was marketed to young kids, the movies were all targeted at kids, and the series aired on primetime on Saturday nights around the same time most other mainstream children's anime gets aired at in Japan. I don't think American kids would be that bothered to have LGBT characters in children's media. Nobody really complained when JK Rowling made Dumbledore gay in Harry Potter in spite of it being a children's book series and no one complained about Korra being in a same sex relationship in the children's animated series, The Legend of Korra. If anything, the biggest complaint was that both of those titles should have been more explicit about the nature of the character's orientations from the start. Regardless, as Adam points out in the blog post, there are other ways they could have handled it more gracefully. I believe it was the Italian dub of Sailor Moon that just decided to make Haruka and Michiru friends and toned down their relationship but they didn't make them cousins either. In fairness to Cloverway, it's still not as bad as the French dub where they oddly decided to change it so that Haruka was a man in his secret identity form and Michiru was just pretending to be Haruka's girlfriend.

I've known a few women who are like Haruka. They're women, they just like to dress and act like guys. And they're usually pretty cool, as Haruka clearly is.

As for the cousins bit...well, that dub was aimed at young kids, and I don't think at the time they factored in whether kids knew about the LGBT community. The original Japanese audience? Sure, they were teens usually. But in America, it was primarily aimed at kids. I remember really thinking they were cousins and not noticing the sexual tension between them because it never occurred to me till much later that sort of relationship could happen. It wasn't until I was in middle or high school and my interest in Sailor Moon was reignited (and by then I knew several people in same-sex relationships) that I saw the subtext. Blew my mind at the time, but now I'm used to it.
